The accountability chart template
Ten starter seats across the five layers, with example outcomes and decision rights. Add the owner's name, then edit every row to match your business.

Why not a classic EOS chart?
The traditional EOS Accountability Chart assumes a stable Integrator/Visionary split and a fixed hierarchy. Real founder-led businesses evolve every quarter. STRATOPLAN treats the chart as a living seat map — versioned, tied to KPIs, and refreshed on every planning cycle.
The five layers
Every STRATOPLAN chart has five layers. Fill them top-down; leave a seat blank if you don't yet have the person. An empty seat is a hiring plan, not a failure.
Ownership
The founders/board seat. Sets vision, capital allocation, and non-negotiables.
Direction (Visionary + Integrator)
Two seats: the person setting the where, and the person running the how. In small teams one human can hold both — but the seats stay separate.
Growth engines
Sales, Marketing, and Product. Each seat owns a north-star KPI (pipeline, MQLs, activation) and a weekly scorecard.
Operations
Delivery, Ops, Finance, People. Each seat owns SLAs, unit economics, and the rhythm of weekly reviews.
Support & Enablement
Legal, IT, Data, Culture. Not always full-time seats — but the accountability must be named.
Filling a seat: the 5-line contract
For every seat, capture exactly five lines. This becomes the seat's contract on the Canvas.
- Seat name — role first, person second. (People change, seats persist.)
- Owner — one human. No shared accountability.
- Purpose — one sentence: why this seat exists.
- Outcomes — 3–5 measurable results with weekly/monthly cadence.
- Decision rights — what this seat can approve without escalation.
Common pitfalls
- Two names in one seat. Kills accountability. Split the seat or pick one owner.
- Titles instead of outcomes. "Head of Growth" isn't accountability. "Pipeline ≥ $2M/qtr" is.
- Locking it forever. Re-open the chart every quarter during MPR — seats evolve as strategy does.
- Ignoring empty seats. An unfilled seat is a strategic bottleneck. Name it, size the hire, and add it to the tracker.
